I think largely will depend on your HR policy, but I’m wondering if a company would require a note from a medical doctor instead of a clinician/therapist. I’ve never been asked to write an article, so I have no real clue, but guess that depends a lot on company policies. However, I think companies have sick days, personal days, and/or vacation days for this reason and a letter from a therapist most likely won’t help you much. As for refusing to write a note? If someone has a good reason to quit because of mental health problems, according to my clinical judgment and a company will accept such a valid letter, I may write, if , in my clinical judgment, I believe there is good reason to write the note, but I will not write a free release from prison if I do not believe the reason is justified.

I don’t know what happened in this situation, but a therapist may refuse to write a letter for many reasons, I think, to answer your question.
